2. Upload Other Applicable Documents using the form to the right: Birth Certificate, Proof of Residency, Report Card, Baptismal Certificate, and Transcripts
3. Submit $100 application fee by either:

cash or cheque by in-person drop-off

by e-transfer (office@stmaurice.mb.ca include student name in memo)
4. Provide the Sending School Questionnaire to your child's current school/daycare (be sure to sign and date the form before providing it to your child's current institution/program ):
